At Ability Plus Therapy, we strive to make every family feel welcomed, valued, and supported. We understand that your time is precious and that consistency is key to effective therapy. That’s why we do our best to schedule the same therapist for each visit, ensuring familiarity and continuity of care.
What is Needed Prior to Scheduling your Initial Visit:
- Patient forms and medical history in our secure patient dashboard
- Prescription
- Insurance information
- Insurance authorization for evaluation, if applicable
- Previous/current Plan of Care, if applicable
What to Bring to Your Initial Visit:
- Insurance card
- Photo ID
- Co-pay or deductible payment
What to Expect During Your Visit:
- Comprehensive Evaluation: Your child will be evaluated by one of our expert therapists. The detailed information you provide in the portal helps us create the most effective care plan tailored to your child’s needs.
- Personalized Plan of Care: Following the evaluation, you will receive a customized Plan of Care with therapy goals and a recommended visit schedule. This plan will be approved by your PCP and insurance, if applicable, and coordinated by our team.

GOOD FAITH ESTIMATE
Under the law, No Surprises Act, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the charges for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
